Landmark Care and Rehabilitation opened in February 2000 and is the premier therapy facility in the community. Landmark Care and Rehabilitation specializes in short term rehabilitation stays following joint replacement surgery, fractures and other surgeries or hospital stays that require therapy for increased strength and mobility.
Landmark Care and Rehabilitation also specializes in post-cardiac care after valve replacement, by-pass surgery or heart attacks. We are able to provide the necessary monitoring and therapy between hospital and home. Hosting a large, specially designed therapy room, Landmark Care and Rehabilitation has state of the art equipment in place to lead the charge in you or your loved ones speedy recovery.
Landmark Care and Rehabilitation also offers care for residents with longer term illnesses and hospice services are available.
You can expect professional, pleasant staff, capable of meeting your health needs. Communication and dignity are high on our lists of "necessary" components of care, and our doors are always open to hear your comments or concerns.
At Landmark Care and Rehabilitation, our goal is to help you or your loved one meet health-related goals and return to maximum potential. In addition to daily communication with our staff, care conferences are held periodically and on request.
Landmark Care and Rehabilitation has 93 beds and all of them are Medicare and Medicaid certified. There are 11 private suites and our double occupancy rooms are large and spacious. Each room has a private bathroom with toilet and sink. Our double occupancy rooms also have an in-room sink. Tours are available at any time and may also be scheduled for a specific time with our Social Services staff.
Our therapy gym is furnished with state of the art Nautilus equipment specifically designed for geriatric clients. This specialized equipment provides the proper and necessary resistance for beneficial strength training. Landmark Care and Rehabilitation's therapy gym is staffed with full-time physical, occupational and speech therapists daily. The personal therapy team develops individualized workout programs for each resident. While focusing on strength and mobility our therapists also concentrate on proper use of equipment and safety awareness. Our therapy program and team are second to none and they help to ensure a safe return to an independent lifestyle.
